Willow Pass Healthcare Center is an 83-bed facility – Specializing in the provision of short-term rehab (inpatient) and long-term care. Our facility is certified by Medicare, Medical, and many HMOs, including Kaiser Permanente to provide 24-hour skilled nursing care.

Our Mission is to provide the highest possible quality of care in a warm, hospitable atmosphere backed by the supporting, caring and dedicated nursing and therapy professionals.

 
 

SCOPE OF SERVICE

We provide patient-centered care with timely and effective comprehensive services. Our goal is to treat the individual needs of each resident and help them achieve their highest possible level of function and independence.

Community Relations

Partnering with Pass Institute & Unitek College for their Nursing students (CNAs & LVNs) clinical rotations.

Community Service

Assisting Walnut Creek Alzheimer’s Association with fundraising for the walk to end Alzheimer’s.
 

Extended Services

We are contracted with major hospice providers such as- Vitas Hospice of the East Bay, Amavi, and Kindred Hospice.
